may not reimburse out-of-plan
treatment of any kind. It
is always best to call your Health Plan first for the
necessary approval. Your insurance
plan is necessary for processing
each claim.
We rely on you to know what your insurance
covers and what it does not.
It
is best to bring your insurance card with you at the
time
of visit. Most of the
information for billing can be obtained
from your card.
