colleagues.

Managed
Care insurance plans (HMO), access to specialty
care requires prior
authorization.
The referral process will
be initiated
at the time of your
regular visit. A
follow-up appointment with my office will
also be arranged for you to best
integrate the sub●specialist's recommendations into your total health care.
