Patients are seen by appointment only. However we
always will "fit you in" in an emergent situation.
On your first visit, we will create a permanent record.
We will ask you to complete a basic registration form.
Please bring your insurance card to every visit.
Try to
arrive a few minutes early for your first visit. If you
have been under care of another doctor, we may ask
your written permission to transfer some of your records.
